Fitness Passport Terms & Conditions

Fitness Passport at Willows Health & Lifestyle Centre allows the Fitness Passport holder:
• Access to the weights zone, cardio zone, and group fitness classes.
• A safe exercising environment free from discrimination and harassment.

Activation Fee

To access the Centre, a digital profile is required. This digital profile also enables you to access the centre, book group fitness classes and use our online member platform. In accordance with Fitness Passport Facility Terms & Conditions, Clause 6 (access card/key/token/other device), the following applies:

1. Who pays: An annual Activation Fee is payable by the Fitness Passport member.

2. Exemption: FP dependants under 18 years who are listed on a parent/guardian’s FP membership are exempt from this fee.

3. Amount: AUD $59.00 incl. GST per member, per year.

4. When payable:
   • On activation (first visit or prior to first class booking) and annually on the anniversary date of activation while the FP membership remains active at Willows.

5. What it covers: Issuance and upkeep of the digital profile (treated as an “other device” under Clause 6) and the systems that enable Centre access, online class bookings, and member-platform functionality.

6. Refunds: Any refunds (if applicable) are handled in line with Clause 6 of the Fitness Passport Facility Terms & Conditions and Willows’ refund procedures.

7. Non-payment: If the fee is unpaid, access to the centre & digital access (including online class bookings) will be suspended until the fee is settled. This does not affect Fitness Passport’s separate membership terms or payments.

Requirements

• If you believe there is a risk to your health by participating in a fitness service at this centre, you must inform Willows Health & Lifestyle Centre in writing about the risk.
• Minimum age is 10 years at the time of signing the membership agreement.
• Members under 18 years (but 10–17 inclusive) require a parent/guardian signature on the membership agreement.
• Shoes, towel, and appropriate clothing are required at all times when using the facility.

Right of Admission

I acknowledge that Willows Health & Lifestyle Centre reserves the right to refuse admission to any person, including members, and may cancel my membership without warning due to inappropriate behaviour, failure to comply with Willows policy, or for any other reason Willows may reasonably determine.

Disclaimer

I acknowledge that whilst on the premises, my person and property are at my own risk. To the extent permitted by law, Willows Health & Lifestyle Centre excludes all liability for any personal injury or damage (whether direct, indirect, special or consequential) suffered by me or loss of property while I am on the premises or arising in any way out of the use of the facilities and equipment provided, however caused (including by Willows’ negligence). Except as provided in these Terms, Willows gives no warranties in respect of the facilities and equipment it provides. I hereby release and will indemnify and keep indemnified Willows Health & Lifestyle Centre for any injury or loss suffered by me while on the premises.
